Maternal Health - Raise Your Voice - Your Participation Needed


We need your help in sharing your unique maternal health journey with researchers who are working to improve maternal care, with a focus on women of color. The research team is comprised of African American health professionals working out of MIT and the VA in Boston. They are in the process of interviewing women to gain insight into their pregnancy, childbirth and post-natal experiences, with the goal of developing solutions that may mitigate the risks of medical complications or barriers to quality care.

Key to NYC: Proof of Vaccination Required to Participate in Indoor Activities

Starting August 17, 2021 people 12 and older will be required to show proof they have received at least one dose of a COVID-19 vaccine authorized for emergency use by the FDA or WHO for:

  • Indoor dining

    • Includes restaurants, catering halls, event spaces, hotel banquet rooms, bars, nightclubs, cafeterias, grocery stores with indoor dining, coffee shops and fast food or quick service with indoor dining

  • Indoor fitness

    • Includes gyms, fitness centers, fitness classes, pools, indoor studios and dance studios

  • Indoor entertainment

    • Includes movie theaters, music and concert venues, museums and galleries, aquariums and zoos, professional sports arenas, indoor stadiums, convention centers, exhibition halls, performing arts theaters, bowling alleys, arcades, pool and billiard halls, recreational game centers, adult entertainment and indoor play areas

This new requirement — called the Key to NYC — includes bars, fitness gyms, movie and stage theatres, museums and other indoor venues. Staff at these locations are also required to be vaccinated.

Proof of Vaccination

Proof of vaccination may include:

If you received the vaccine outside the U.S., you must have an official immunization record that includes:

  • First name and last name

  • Date of birth

  • Vaccine product name (only vaccines authorized by the WHO are acceptable)

  • Date(s) administered

  • Site where the vaccine was administered, or name of the person who administered it

CANCELLATION AND REFUND POLICY

IN-STUDIO CLASSES

  • Cancellations for all in studio classes must be made at least 12 hours before the start of class. Students that do not attend the class will not receive a refund or credit.

  • All cancellations may be made on Mindbody, the Fit4Dance App, via voicemail 347.921.2404, or via email to info@fit4dancenyc.com before the class start time. Fit4Dance is committed to keeping our community safe. If a student is feeling ill or experiencing any COVID-19 symptoms please email info@fit4dancenyc.com in order to cancel or reschedule a class.

  • Fit4Dance reserves the right to cancel a class should the class have an enrollment of less than five students. Students will be notified at least 3 hours prior to the start of class if there is a cancellation. Please make sure your contact information is up to date when you register for classes. If Fit4Dance cancels a class due to low enrollment, the student will receive a credit to their account. No refund will be given.


COVID-19 Refund Policy

  • In the event of studio closure due to COVID-19, Fit4Dance will continue programming online. No refunds will be given for programming changes due to COVID-19 such as, but not limited to travel restrictions, classes shifted from In-Studio to Online distance learning, substitute teachers, and any school closure due to a positive test result.

  • If Fit4Dance must cancel classes due to COVID-19, we will issue credit for future classes. No refunds will be given.


MAKEUP POLICY DUE TO INTERNET ISSUES

  • Internet Issues on Fit4Dance Side: If the teacher has problems with internet connectivity issues more than twice and cannot make up time by adding additional time at the end of the class, a credit will be awarded for that single class to the student’s MindBody account for future use.

  • Internet Issues on the student side: If you have signed up and paid for a class, but cannot attend due to bad internet connection, we will offer you a credit to reschedule the class. If your internet connection drops during a class more than twice and you cannot complete class, please notify us at info@fit4dancenyc.com and we will offer a class credit.
    PLEASE NOTE: If a student attends 15 minutes or more of a class no class credit will be given
